I've found that it's mostly the author's responsibility to fact check. I wish that wasn't the case but it is.
Someone said that they look at the dates of sources used in a bibliography and discount books w/earlier dates. I've found that the best resources are ones taken from the time period I'm writing about. If I"m writing about 1945 then there's nothing better than a good news article or book from that period. Science, of course, is a different thing and in that case of course you don't want to see outdated material in a bibliography. But I can't think of another example besides science based books.
