I believe that the Malfoys fell out of favor because Mr. Malfoy had also failed Voldemort several times -- allowing Harry to slip thorugh his fingers-- and especially in the Ministry of Magic in book 5 when he failed to recover the prophecy, Narcissa resented Voldemort because of how he used their son and then in addition Draco failing to kill Dumbledore was the final thing.

I thought the Malfoys were out of favor with Voldemort because Draco was unable to kill Dumbledore as he had been instructed to do. I also think I read that Luna ended up with Dean, but I'm not sure of that.
