I just called the National Book Foundation to clarify this: If a National Book Award judge wishes to consider a book that has NOT been nominated by a publisher, that judge asks the National Book Foundation to request that the publisher nominate the book, and the publisher sends in the book to the Foundation with the $100 fee.
This has occurred on several occasions.
